The inventory targets which habits need to be addresses. There is a tip sheet that cooresponds to each area of the inventory. Use this inventory and handouts in whatever way works best for your practice. Tip Sheets can stand alone to deliver a healthy message by making it available with other information in the waiting area of your office or you can reinforce the message when you talk to families. Health Education Services SmartChoice is a medically supervised health education program offered to employees who are insured by the four largest employer groups in Collier County (School District, Government, Sheriff's Office and NCH) This is a covered benefit and no out of pocket expense. It targets those who have, or are at risk for chronic diseases such as heart disease, high blood pressure, diabetes, high cholesterol, asthma and weight management. SmartChoice focuses on education, prevention, behavior modification, and self-management to minimize common health risks. Intended for adults, hundreds of participants have found that it benefits the whole family! Immokalee office of the Collier County Health Department provides a Stop Smoking Class. The 5-week class can be provided at your convenience. The cost is $10.00 and includes nicotine patches.
