Fit & Fun Family Habit Inventory  

This inventory can be given to clients as they check in and reviewed at your consultation. The inventory targets which habits need to be addresses. There is a tip sheet that cooresponds to each area of the inventory.

Use this inventory and handouts in whatever way works best for your practice. Tip Sheets can stand alone to deliver a healthy message by making it available with other information in the waiting area of your office or you can reinforce the message when you talk to families.

Suggested use of this tool kit for the most effective outcomes:

  • In the waiting area or exam room, have a family member complete the Family Habit Inventory.
  • Use the Family Habit Inventory to assess and discuss current family habits and readiness for change. The Family Habit Inventory asks questions that are directly linked to information provided on each Family Tip Sheet so that you can easily identify which Family Tips will be most helpful.
  • Provide Family Tip Sheets and Kids Activity Pages as appropriate. Use ideas on the Health Care Professional Tip Sheets to reinforce the message.
  • Follow-up by repeating the Family Habit Inventory at intervals you deem appropriate.
  • Set-up a supportive office environment. Use ideas in the Health Care Professional Tip Sheets to reinforce the healthy messages. To create a strong emphasis on one healthy behavior at a time, alternate the use of suggested displays and interactive ideas throughout the year.

Get everyone involved and show families and kids that healthy choices CAN be appealing!

Health Education Services

SmartChoice is a medically supervised health education program offered to employees who are insured by the four largest employer groups in Collier County (School District, Government, Sheriff's Office and NCH) This is a covered benefit and no out of pocket expense. It targets those who have, or are at risk for chronic diseases such as heart disease, high blood pressure, diabetes, high cholesterol, asthma and weight management.  SmartChoice focuses on education, prevention, behavior modification, and self-management to minimize common health risks.  Intended for adults, hundreds of participants have found that it benefits the whole family!      Free Diabetes Support Group meetings every month:

NCH Diabetes Self Management Classes -   Learn about the preventative measures to maintain a healthy blood sugar level. These classes offer a free glucometer and free wellness membership along with the program. Classes are located at both the North and South Wellness Centers and there is a reduced rate for spouses. A complimentary high carbohydrate breakfast will be served.  Blood sugar will be re-checked after the seminar. YOU MUST FAST FOR 12 HOURS.  For information on classes call 436-6755.

Smoking Cessation Classes

Immokalee office of the Collier County Health Department provides a Stop Smoking Class.  The 5-week class can be provided at your convenience.  The cost is $10.00 and includes nicotine patches.  Businesses or agencies interested in hosting a class at your worksite are welcome (Immokalee area only).   For more information, please call Collier County Health Department at 658-7300.

Workouts

Community resource page has specific workouts at area parks in the north, south, east and west ends of Collier County. They cover beginner, intermediate and advanced levels and were designed to give patients while counseling them so that compliance is increased.

Diabetes Education: This award-winning Link for Life tool uses computer animation to define diabetes and explain key components of the disease. It is VERY easy, good for low literacy and available in English and Spanish. The Link for Life animation was supported through an educational grant from GlaxoSmithKline.
